Etkö pysty nielemään kaikkea ruokaa?

Jos limakalvo on tulehtunut, sen saattaa huomata syödessä. Yleisimmät oireet ovat kipu niellessä tai ruoan hidas kulkeutuminen mahalaukkuun.1

Toisinaan ruoka saattaa jäädä kiinni matkalla mahalaukkuun. Joskus auttaa, jos nielee runsaasti vettä, mutta joissakin tapauksissa takertuneen ruoan irrottamiseen vaaditaan sairaalassa käyntiä.

Monet tottuvat ongelmaan ja yrittävät välttää vaivoja pilkkomalla ruoan hyvin pieniksi paloiksi, pureskelemalla huolellisesti, juomalla runsaasti ja välttämällä kovaa leipää, riisiä ja lihaa. Tämä saattaa viivyttää diagnoosin tekemistä ja johtaa pitkäaikaiseen kärsimykseen.

Mikä on eosinofiilinen ruokatorvitulehdus?

Eosinofiilinen ruokatorvitulehdus on diagnoosi, jonka esiintyvyysluvut ovat kasvussa. Eosinofiilisessa ruokatorvitulehduksessa ruokatorven sisällä oleva limakalvo tulehtuu. Jos tulehdus jatkuu useita vuosia, se saattaa johtaa esimerkiksi haavaumiin, kaventumiin ja arpeutumiseen.

Eosinofiilisen ruokatorvitulehduksen aiheuttaja ei ole tarkkaan tiedossa. Se tiedetään, että sairauden saattavat laukaista jotkin ruoka-aineet. Joskus allergisen reaktion ruokatorvessa aiheuttaa ruoka-aineiden yhdistelmä. On tärkeää tietää, että allergia on vain paikallisesti ruokatorvessa, eikä mikään veri- tai ihoärsytyskoe osoita allergian aiheuttajaa ruokatorvessa.

Potilaissa on kaikenikäisiä, mutta lasten ja aikuisten taudin oireissa on eroavaisuuksia. Miehet sairastuvat naisia useammin ja sama pätee lapsiin: pojat sairastuvat useammin.

Jos eosinofiilista ruokatorvitulehdusta ei hoideta, sairaus saattaa pahentua. Jos sinulla on eosinofiiliseen ruokatorvitulehdukseen viittavia oireita tai vaivoja, keskustele asiasta lääkärisi kanssa.1

Mitä ruokatorvessa tapahtuu?

Ruokatorvi on putkimainen elin, jonka sisäpuolella on limakalvo. Limakalvo on kostea ja liukas, jotta ruoka pääsisi helpommin kulkeutumaan mahalaukkuun.

Eosinofiilisessa ruokatorvitulehduksessa limakalvo tulehtuu, mikä johtaa siihen, että ruoka ei pääse kulkeutumaan mahalaukkuun tai että se takertuu kiinni matkalla sinne.1

Eosinofiilisen ruokatorvitulehduksen oireet

Eosinofiilisen ruokatorvitulehduksen yleisimpiin oireisiin kuuluvat kipu niellessä, nielemisvaikeudet sekä ruoan hidas kulkeutuminen mahalaukkuun. Potilas tottuu usein oireisiin ja kehittää strategioita niiden välttämiseksi. Ruoka pilkotaan hyvin pieniksi paloiksi ja pureskellaan huolellisesti, aterioiden yhteydessä juodaan runsaasti ja tiettyjä ruoka-aineita kuten kovaa leipää, riisiä ja lihaa vältetään.

Se, ettei kykene syömään jotakin ruoka-ainetta tai yhtä nopeasti kuin muut, voi tietenkin heikentää elämänlaatua. Oireet voivat tulla ja mennä ja ne voivat vähentyä hetkeksi ja tulla sitten takaisin ja pahentua ajan myötä.

On tärkeää kertoa oireista lääkärille, koska diagnoosin tekemiseen tarvitaan tietoja terveydentilastasi.1

  • Nielemisvaikeudet (dysfagia)

  • Rintakipu
  • Ruoan takertuminen ruokatorveen


Oireet, jotka saattavat viitata eosinofiiliseen ruokatorvitulehdukseen:

  • Närästys

 

Lapset

Eosinofiilisen ruokatorvitulehduksen oireet lapsilla:

  • Happamat röyhtäisyt
  • Pahoinvointi ja oksentaminen
  • Vatsakipu
  • Syö hyvin hitaasti tai kieltäytyy syömästä
  • Kasvuhäiriöt
  • Ruoka takertuu ruokatorveen

Diagnoosi ja hoito

Voidakseen tehdä oikean diagnoosin lääkäri kysyy aluksi oireista, niiden ilmenemisestä sekä siitä, miten paljon ne vaikuttavat elämänlaatuun. Tärkeitä tietoja ovat myös aikaisemmat sairaudet, mahdolliset allergiat ja lääkitys. Jos epäillään eosinofiilista ruokatorvitulehdusta, tehdään gastroskopia ja otetaan myös koepaloja.

Joissakin tapauksissa gastroskopian yhteydessä tutkittaessa ruokatorvea kameralla voidaan nähdä selkeitä merkkejä eosinofiilisesta ruokatorvitulehduksesta. Potilaista 75 prosentilla ruokatorvi näyttää normaalilta, joten diagnoosin vahvistamiseksi koepalojen ottaminen on tarpeen..

Eosinofiilista ruokatorvitulehdusta hoidetaan lääkkeillä. Oikean hoidon määrittelemiseksi tarvitaan oikeaa diagnoosia. Kun diagnoosi on tehty, on olemassa suosituksia siitä, miten sairautta hoidetaan ja seurataan.
